The Human Services Division of the Northern Kentucky Area Development District is seeking nominations for Outstanding Achievement in the Human Services Field. Nominations will be accepted for either program (or project) and professional. However, only one award will be presented at the Northern Kentucky Area Development District Annual Meeting and Awards Ceremony. All nominations must be submitted by June 30, 2011.

The NKADD Annual Meeting is August 15, 2011. Criteria for the awards are listed below.

• Program (or project)is affiliated with the non-profit human services field
• Program (or project) provides a service or impacts at least one of the eight (8) counties of the Northern Kentucky Area Development District (Boone, Campbell, Carroll, Gallatin, Grant, Kenton, Owen or Pendleton Counties)
• Program (or project) provides services for which a gap has been identified

• Nominee is involved in the non-profit human services field
• Nominee provides a service or impact in at least one of the eight (8) counties of the Northern Kentucky Area Development District (Boone, Campbell, Carroll, Gallatin, Grant, Kenton, Owen or Pendleton Counties)
• Nominee provides exceptional leadership in his or her own organization in the Northern Kentucky community
