The Long Term Care Division provides innovative and quality services, delivered in a variety of settings while preserving the dignity and choice of our clients. Our professional staff provide specialized case management services both in the fields of geriatries and disabilities.

Geriatric Case Management
Provided to older adults aged 60 and over who receive services funded through the Older American’s Act, State of Kentucky and private Fiscal Courts. In addition to direct services our case managers are all certified as State Health Insurance Program (SHIP) counselors and provide assistance to our clients with Medicare, Medicaid and private insurances. By assigning our case managers to geographic areas they are able to develop strong ties to local communities and provide linkage and referrals for other services. We work closely with family members offering support and services which address the needs of the caregivers and help our clients remain in their homes.

Consumer Directoed Option (CDO)
Support Brokers provide case management and financial services to members receiving Medicaid Waiver services. We currently serve a population ranging in age from birth to 101. Because of the age and diversity of our population we have developed a team with expertise in medical, educational, adult work training and aging issues. This team is able to provide the continuum of care needed to assist our members reach and maintain their full potential.

Commodity Supplemental Food Program (CSFP)
The Long Term Care Division is designated by the Kentucky Department of Agriculture as the Food Bank serving Northern Kentucky. As the Food Bank we purchase and distribute food commodities for low income and/or aged through the United States Department of Agriculture (USDA). With the network of local food pantries and soup kitchens we work in conjunction to assure low income individuals in Northern Kentucky have access to nutritious foods.
