SERVICES AVAILABLE FOR SENIORS
ADULT DAY SERVICES-This program provides a supportive environment for individuals who cannot remain at home alone. Programs provide licensed supervision of the participant’s medical and health needs. Socialization, activities, meal and snacks are provided. Serves as a source of respite for caregivers. (859)-282-2700
CASE MANAGEMENT-The Case Management Unit serves as a single entry point into a variety of supportive services for older adults who have multiple needs. FAMILY CAREGIVER-Support and assistance to family caregivers providing care to individuals 60 and older and caregivers over 60 who are the primary caregiver to minor children. IN-HOME SERVICES-Provided for at-risk older adults designed to enable them to live independently. Services include assessment and case management, personal care, homemaker, home delivered meals, home repairs, escort services and respite care. (859)282-2700
LEGAL SERVICES-Legal assistance is available for those 60 and older for problems with Medicare, Medicaid, food stamps, utilities cut off, housing issues, family issues and grandparent rights. Limited power of attorney and guardianship counseling. (859)431-8200
NUTRITION PROGRAMS- Home-delivered meals assure that homebound elderly receive adequate nutrition and some regular personal contact. With increasing numbers of frail elderly and shortened hospital stays, home-delivered meals have become a critical component of the growing community service system. Hot, mid-day meals are provided to older adults, 60 and older, at local senior centers and/or nutrition sites. Meals are provided at no charge, but donations are accepted. For more information regarding meals and daily activities, call individual centers and/or sites. Personal Care Attendant Program (PCAP)-Established by the Kentucky General Assembly, this program mandates personal care attendant services for severely physically disabled adults age 18 and over who are at risk of institutional care, providing them the opportunity to live within their homes and communities. The client must be mentally alert with permanent, temporary or recurring functional loss of two or more limbs. The disabled adult has the management responsibility of hiring, supervising and firing the attendant. This service allows the adult to achieve greater physical independence. http://www.cilo.net/
